Preparing for Your First Appointment at CareCo Physio and Massage
We want every patient to feel informed and comfortable before their first visit. Here’s everything you need to know:
At all appointments, wear comfortable clothing that allows easy movement. Athletic wear, loose pants, shorts, or a fitted top work well, depending on the area being treated. We also recommend supportive footwear. Athletic shoes are ideal.
New patients will need to complete our comprehensive intake forms, which help us better understand your medical history, current concerns, and personal goals so we can craft the best care plan for your needs. You can complete forms before arrival or arrive 10-15 minutes early to complete them in the clinic.
Please bring the following with you on your first appointment:
- A government-issued identification
- Any relevant imaging reports
- A list of current medications, if applicable
- A copy of a physician’s referral, if applicable. You can also send this to ahead of time.
